Role Overview

Provide specialist social work support to children and young people with complex and/or lifelong disabilities, and their families. Ensure safety, development, and inclusion via assessment, planning, direct intervention, coordination with multi‑agency partners, and robust case management.

Key Responsibilities

• Conduct comprehensive assessments to identify children's strengths, needs, risks, and safeguarding concerns.

• Develop and implement person‑centred care/support plans, including short‑break services.

• Undertake direct work: one‑to‑one sessions, group support, and community outreach.

• Coordinate multi‑agency collaboration (health, education, therapy, adult services).

• Act as advocate for children's rights and access to specialist services.

• Manage a caseload effectively, maintain accurate documentation for case reviews and audits.

• Maintain up-to-date records and adhere to legislation and best-practice frameworks.

Essential Qualifications & Experience

• Qualified Social Worker (DipSW, CQSW, BA Hons Social Work, or equivalent) with registration (Social Work England or equivalent).

• Post-qualifying experience working with children with disabilities and their families.

• Enhanced DBS check.

• Knowledge of relevant legislation, safeguarding, and care planning practices.

• Strong communication, interpersonal, advocacy, assessment, and organisational skills.
