As an Adult Social Worker in the CHC Team, your key roles and responsibilities will include:
Conducting comprehensive assessments of individuals' health and social care needs to determine eligibility for NHS Continuing Healthcare funding.
Working collaboratively as part of a multi-disciplinary team (MDT) to review and assess complex cases, including participation in MDT meetings and best interest meetings.
Applying National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care guidelines consistently and accurately in all assessment and decision-making processes.
Liaising effectively with individuals, their families, carers, and a wide range of professionals (e.g., healthcare providers, care home staff, GPs) to gather information and ensure holistic care planning.
Developing and reviewing person-centred care plans that address identified needs and promote independence and well-being.
Advocating for individuals to ensure their voice is heard and their rights are upheld throughout the CHC assessment and review process.
Managing a caseload of CHC assessments and reviews, prioritizing effectively to meet deadlines and service demands.
Preparing clear, concise, and analytical reports and documentation to support funding decisions and review processes.
Identifying safeguarding concerns and taking appropriate action in line with Newcastle Council's safeguarding policies and procedures.
Providing advice and guidance to individuals and families regarding the CHC process and available support options.
Maintaining accurate and up-to-date case records in compliance with professional standards and organisational requirements.
Key Requirements:
You must be a Qualified Social Worker with a degree in Social Work or equivalent.
Social Work England registration is essential and must be current.
An enhanced DBS check will be required for this role.
Previous experience in adult social care, particularly with assessment or Continuing Healthcare processes, is highly desirable.
