deputy team manager - hospital discharge (transfer of care).

We are looking for an experienced Deputy Team Manager to support a fast-paced Hospital Discharge Team in Southwark. This role focuses on safe, timely discharges and transfer of care planning, working closely with health colleagues to prevent delays and ensure effective outcomes.

Key Responsibilities:

• Supporting the Team Manager with supervision, allocation, and oversight of complex cases

• Leading on discharge planning in line with the Discharge to Assess model

• Overseeing Care Act assessments, safeguarding, and Mental Capacity Assessments

• Liaising with hospital teams and community services to enable safe discharge

• Ensuring quality practice and compliance with statutory responsibilities

Requirements:

• SWE registration

• Strong background in hospital discharge or transfer of care

• Confident in managing staff and high-pressure situations

• Solid knowledge of the Care Act, MCA, and Continuing Healthcare processes
