Darlington Council is seeking a dedicated and compassionate individual to join our Hospital Discharge Team. This crucial role plays a vital part in ensuring smooth transitions for patients leaving hospital and supporting their continued care in the community. We offer a competitive hourly rate of £26.26.

Responsibilities will include, but are not limited to: coordinating patient discharges, liaising with hospital staff, social workers, and family members, arranging transport and home care services, and providing ongoing support to patients to facilitate a successful transition home.

The ideal candidate will possess excellent communication and interpersonal skills, strong organizational abilities, and the ability to work effectively both independently and as part of a team. Experience in a healthcare setting is highly desirable, as is a working knowledge of relevant social care legislation.
