Newcastle Council is seeking an enthusiastic individual to join our Hospital Discharge Team. This is an excellent opportunity to make a real difference in people's lives, ensuring safe and effective transitions from hospital back into the community.
About the Role:
As a Qualified Social Worker in the Hospital Discharge Team, you will play a crucial role in assessing, planning, and coordinating discharge pathways for patients. You will work collaboratively with healthcare professionals, patients, families, and external agencies to ensure holistic and person-centred care.
Key Roles & Responsibilities:
- Conducting comprehensive social care assessments for patients within a hospital setting to determine their needs for safe discharge.
- Developing and implementing robust care plans in partnership with patients, their families, and multidisciplinary teams.
- Facilitating timely and appropriate discharges, considering housing, care, financial, and support needs.
- Working collaboratively with health colleagues, including doctors, nurses, and therapists, to ensure integrated discharge planning.
- Liaising with external agencies and community services to arrange ongoing support packages.
- Providing information, advice, and advocacy to patients and their families regarding their rights and available services.
- Managing a caseload of diverse and complex discharge cases, prioritizing effectively.
- Maintaining accurate and up-to-date records in line with council policies and professional standards.
- Participating in multidisciplinary team meetings and contributing to service development.
- Adhering to all relevant legislation, policies, and procedures, including the Care Act 2014.
Key Requirements:
- You must be a Qualified Social Worker.
- Social Care England registration is essential.
- An Enhanced DBS (Disclosure and Barring Service) check is required and must be updated.
