We are seeking a skilled and motivated Adult Social Worker to join the Hospital Discharge Team in Newcastle.
The role involves working in a fast-paced, multidisciplinary environment to ensure timely and safe discharges from hospital to home or appropriate care settings.
This is a crucial front-line role aimed at reducing delayed discharges, promoting independence, and safeguarding vulnerable adults.
Key Responsibilities
Undertake Care Act 2014 assessments and reviews for patients being discharged from hospital.
Work closely with NHS colleagues, hospital discharge coordinators, therapists, and families to create safe discharge plans.
Conduct safeguarding enquiries when appropriate.
Assess mental capacity and complete best interest decisions where necessary.
Arrange care and support services in line with eligibility criteria and funding panels.
Update case notes accurately using the council's electronic case management system.
Ensure timely interventions and contribute to hospital flow and system pressures.
Essential Requirements
Degree in Social Work (or equivalent qualification).
Registration with Social Work England.
Previous experience in hospital discharge or adult social care settings.
Good knowledge of the Care Act, Mental Capacity Act, and hospital discharge guidance.
Enhanced DBS on the update service (or willingness to apply).
Strong organisational, communication, and multidisciplinary working skills.

Full-time hours - 37 hours per week.
Hybrid working available (1-2 days in the office/hospital).
