We're seeking a dedicated professional to join our dynamic LDA Team in Newcastle. This is your chance to work in a supportive, multidisciplinary environment, delivering crucial, person-centered care to adults with learning disabilities and/or autism.
What You'll Be Doing:
Person-Centered Planning: Conduct comprehensive Care Act 2014 assessments to identify needs and co-produce innovative support plans with service users and their families.
Empowerment & Advocacy: Advocate for the rights of vulnerable individuals, promoting their independence and ensuring they are at the heart of all decision-making processes.
Collaborative Working: Partner with a wide range of professionals, including health professionals, housing providers, and advocacy services, to ensure a holistic approach to care.
Safeguarding & Legislation: Undertake risk assessments, safeguarding investigations, and apply your expertise in key legislation like the Mental Capacity Act and DoLS.
Transitions & Support: Support seamless hospital discharges and transitions into community-based services, helping individuals thrive in their own homes.
What We're Looking For:
A qualified Social Worker with Social Work England registration.
Demonstrable experience working with adults with learning disabilities, autism, and/or complex mental health needs.
A strong understanding of relevant legislation and best practice.
Excellent communication, organizational, and analytical skills.
A genuine commitment to inclusion and rights-based practice.
