Pay Rate: £37 per hour
We have an exciting opportunity for you to join our team at Cumberland Council, specializing in Learning Disabilities, Transitions, and Autism (LDTA). Roles & Responsibilities:
Assessment & Care Planning:
Conduct comprehensive needs-led assessments for individuals with learning disabilities and autism, identifying strengths, needs, and risks.
Develop, implement, and review personalized care and support plans to promote independence, choice, and well-being.
Case Management & Advocacy:
Manage a complex caseload, ensuring all statutory duties are met in line with the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation.
Act as an advocate for service users, ensuring their voices are heard and their rights are protected.
Multi-Agency Collaboration:
Work collaboratively with health professionals, housing providers, education services, and other key partners to deliver holistic and coordinated support.
Facilitate seamless transitions for young people with disabilities from children's to adult services.
Safeguarding & Risk Management:
Actively participate in safeguarding inquiries, ensuring the safety and protection of vulnerable adults.
Conduct risk assessments and develop effective strategies to mitigate risks, promoting a safe environment for service users.
Key Requirements:
You must be a Qualified Social Worker.
Active registration with Social Work England is essential.
A valid and updated Disclosure and Barring Service (DBS) check is required.
