Key Responsibilities:
Complete Care Act assessments and reviews.
Undertake safeguarding enquiries and risk assessments.
Develop and review care and support plans tailored to individual needs.
Work closely with health professionals, housing officers, and community organisations.
Maintain accurate case records using the council's electronic case management system.
Support timely hospital discharges and community-based interventions.
Requirements:
You must be a Qualified Social Worker with a degree in Social Work or equivalent.
Registration with Social Work England is essential.
You should have recent experience working in adult services, ideally in a locality or community team.
Strong knowledge of the Care Act 2014, safeguarding procedures, and the Mental Capacity Act is required.
An enhanced DBS on the update service (or willingness to obtain one) is necessary.
Strong communication, assessment, and organisational skills are essential.
Contract Details:
Hourly rate: £19-£25 per hour (umbrella), depending on experience.
Full-time: 37 hours per week.
Initial 3-month contract with likely extension.
Hybrid working: Minimum 3 days per week office-based in Gateshead.
