As an Adult Social Worker in the Locality Team, your key roles and responsibilities will include:
Conducting holistic person-centred assessments for adults with diverse needs, including older people, individuals with physical disabilities, learning disabilities, and mental health conditions.
Developing and reviewing bespoke care and support plans in collaboration with individuals, their families, and carers, focusing on promoting independence, choice, and control.
Facilitating access to a range of community-based services and resources, ensuring individuals receive timely and appropriate support.
Working collaboratively with multi-agency partners, including health professionals, housing, voluntary organisations, and other council departments, to deliver integrated care.
Identifying and responding to safeguarding concerns for vulnerable adults, undertaking Section 42 enquiries as required and ensuring all actions are in line with safeguarding policies and procedures.
Undertaking Mental Capacity Act (MCA) assessments and acting in individuals' best interests where necessary, ensuring their rights are protected.
Managing a diverse caseload, prioritising effectively to meet statutory requirements and service demands.
Preparing accurate, analytical, and concise reports and documentation for case conferences, reviews, and other professional meetings.
Providing expert advice and guidance to individuals, families, and other professionals regarding adult social care services and eligibility criteria.
Adhering to all relevant legislation, statutory guidance, and local policies to ensure high standards of professional practice.
Key Requirements:
You must be a Qualified Social Worker with a degree in Social Work or equivalent.
Social Work England registration is essential and must be current.
An enhanced DBS check will be required for this role.
Demonstrable experience working with adults in a social care setting is highly desirable.
