Are you an experienced practitioner with a deep understanding of Continuing Healthcare (CHC)? We are looking for a dedicated professional to join our team in Carlisle (with hybrid flexibility) to manage a specific caseload of assessments.
This role sits within Adult Social Care, offering the unique opportunity to apply your expertise to complex adult cases while occasionally supporting our Children's teams.
Role Overview
Rate: £500 - £600 per day (DOE)
Contract: Initial 3-month placement (high potential for extension)
Location: Carlisle (Preferred), with Hybrid working options available
Capacity: Full-time (As required to fulfill assessments)
Caseload: Focused delivery of 25-30 assessments
Key Responsibilities
You will be responsible for navigating the multidisciplinary assessment process to determine eligibility for NHS Continuing Healthcare. This is not a DfE-regulated role but requires a high level of clinical or social care judgment.
Conduct comprehensive assessments for adults with primary, complex, intense, or unpredictable health needs.
Work collaboratively across multidisciplinary teams (MDT) to ensure care packages are fully funded and appropriate for home or residential settings.
Maintain accurate documentation to support funding decisions based on the National Framework for NHS CHC.
Support Children's services on an ad-hoc basis, providing a cross-continuum approach to care assessment.
Requirements
Subject Matter Expertise: Proven experience as an NHS CHC Assessor.
Clinical/Social Care Background: Deep knowledge of the distinction between healthcare and social care needs.
Adaptability: While primarily an Adult Social Care role, you should be comfortable pivoting to support Children's teams when required.
Efficiency: Ability to manage a defined volume of assessments (25-30) within the initial 3-month window.
