About the Role:
We are seeking a dedicated Social Worker to join our dynamic multi-disciplinary team, providing crucial support to individuals recovering from neurological conditions. In this role, you will play a vital part in:
- Conducting comprehensive social work assessments for individuals with neurological conditions, focusing on their social, emotional, and practical needs.
- Developing and implementing personalised care plans in collaboration with service users, families, and other rehabilitation professionals.
- Providing emotional support, guidance, and advocacy to service users and their families navigating the complexities of neurological rehabilitation.
- Connecting individuals with appropriate community resources, support networks, and long-term care solutions.
- Working collaboratively within a multi-disciplinary team (including physiotherapists, occupational therapists, speech and language therapists, and nurses) to ensure holistic and integrated care.
- Safeguarding vulnerable adults and ensuring compliance with all relevant legislation and best practice guidelines.
About You:
You will be a Qualified Social Worker with:
- Experience working with adults, ideally within a rehabilitation, disability, or health setting.
- A strong understanding of the impact of neurological conditions on individuals and families.
- Excellent assessment, communication, and organisational skills.
- A commitment to promoting independence and enhancing the quality of life for service users.
- Current Social Work England registration.
Rate: £32.00 Umbrella
