Continuing Healthcare (CHC) Social Worker - Sefton
We're currently recruiting for an experienced Social Worker to join the Continuing Healthcare (CHC) Team within Sefton Council.
This is a great opportunity to be part of a collaborative and supportive team working across health and social care, ensuring adults receive the right support and funding based on their assessed needs.
The role involves working closely with health colleagues, service users, and families to complete assessments under the Care Act 2014, contribute to CHC eligibility processes, and develop person-centred support plans that promote independence and wellbeing.

Role Overview:
- Undertake Care Act 2014 assessments and reviews
- Contribute to Continuing Healthcare (CHC) assessments, DST meetings, and joint funding discussions
- Work collaboratively with NHS colleagues, families, and partner agencies
- Develop person-centred, outcome-focused support plans
- Promote wellbeing, choice, and independence for adults
- Ensure timely and accurate recording in line with statutory duties
 
 
To be considered for the role, ideally you will have:
- Qualified Social Worker (DipSW / Degree / CQSW) with SWE registration
- Experience working in Adult Social Care, ideally within Continuing Healthcare or hospital/community teams
- Strong understanding of the Care Act 2014 and Mental Capacity Act 2005
- Experience of CHC processes and multi-agency working
 Excellent assessment, communication, and organisational skills
