We are recruiting two experienced Locum Social Workers to join an Urgent Care Hospital Discharge Team in Trafford, Greater Manchester. These roles require practitioners confident in completing timely assessments, supporting safe discharges, and working within a fast-paced, multi-disciplinary environment.

- Assess the care and support needs of adults and carers across a range of settings, working in partnership to plan effective interventions
- Complete Mental Capacity Assessments and lead/participate in Best Interest Meetings in line with the Mental Capacity Act 2005
- Support adults lacking capacity around accommodation and care decisions, including applications to the Court of Protection for deprivation of liberty authorisations
- Manage a varied caseload of adults with care and support needs, including adults at risk of harm or neglect
- Identify and respond to individuals who are vulnerable or at risk of significant harm, working within safeguarding frameworks and supporting the right to self-determination
- Carry out assessments as part of a multi-disciplinary team, contributing to safe and timely hospital discharge planning
- Liaise effectively with health colleagues, social care teams, and partner agencies
- Maintain accurate, up-to-date records and provide required data and documentation
- Advocate for adults to resolve conflict and ensure their wishes, needs, and rights are represented
To be considered for the role, ideally you will have:
- Experience in Hospital Discharge / Urgent Care or a similar fast-paced adults' service
- Strong knowledge of MCA, Best Interests, DoLS/CoP processes
- Confident in safeguarding, assessment, MDT work, and case management
- SWE registration and Social Work qualification
