Social Workers - Integrated Community Coordination Teams (ICCTs), Wirral
We're currently recruiting on behalf of Wirral Council, who are looking for both Social Workers and Experienced Social Workers to join their Integrated Community Coordination Teams (ICCTs).

You'll work collaboratively within a multi-disciplinary team to support adults with a range of needs - helping them to build on their strengths, access community resources, and maintain their independence.

Responsibilities:
- Carry out Care Act 2014 assessments and reviews
- Develop strengths-based, person-centred support plans
- Promote wellbeing, choice, and independence
- Build effective relationships with health partners, families, and local services
- Work within Wirral's 3 Conversations (3Cs) model
To be considered for the role:
- Qualified Social Worker (DipSW / Degree / CQSW) with SWE registration
- Previous experience in Adult Social Care (1-3 years preferred)
- Good working knowledge of the Care Act 2014
- Commitment to strengths-based, person-centred practice
