Role:
- Strength-based reviews
- Supporting individuals in Extra Care settings
- Promoting independence and positive outcomes
- Multi-disciplinary working
- Delivering person-centred practice
To be considered for the role, ideally you will have:
- Registered with Social Work England
- Have experience within Adult Social Care
- 2 years + experience of working within similar team
- Be confident completing reviews and managing caseloads
- Work effectively within multi-agency environments
- Demonstrate strong communication and organisational skills
