Job Overview:
Southwark Council is seeking a capable and experienced Deputy Team Manager to join the Hospital Discharge (Transfer of Care) Team. The successful candidate will support the Team Manager in overseeing the delivery of safe and timely hospital discharges, ensuring best outcomes for adults and adherence to statutory requirements.
Key Responsibilities:
Support in leading a multidisciplinary team managing complex discharges from acute hospitals.
Oversee allocation of work, case audits, performance monitoring, and day-to-day team operations.
Provide supervision and mentoring to social workers and other practitioners in the team.
Ensure all discharges comply with Care Act 2014, Discharge to Assess (D2A) guidance, and local safeguarding procedures.
Liaise with health partners, care providers, and internal teams to coordinate seamless discharge planning.
Assist in developing and implementing service improvements and efficiencies.
Act as a point of escalation for complex or high-risk cases.
Requirements:
Qualified Social Worker registered with Social Work England.
Proven experience in hospital discharge or adult social care settings.
Previous experience in a supervisory or leadership role is essential.
Sound understanding of the legislative framework around adult social care, hospital discharge, and safeguarding.
Strong communication, decision-making, and conflict-resolution skills.
Ability to work under pressure in a fast-paced, multi-agency environment.
