About the Role
We are seeking a motivated and compassionate Social Worker to join our dedicated Adult Services team based in Haddington.
This is a fantastic opportunity to work within a supportive community setting, focusing primarily on carrying out assessments under the Self-Directed Support (SDS) legislation. You will play a crucial role in empowering adults to make choices about their care and support, enabling them to live independent and fulfilling lives within the East Lothian community.
Key Responsibilities
- Self-Directed Support (SDS) Assessments: Conducting comprehensive person-centred assessments to determine eligibility for support, focusing on needs, strengths, and desired outcomes.
- Care Planning: Working collaboratively with individuals, families, carers, and other professionals to develop tailored support plans across the four SDS options.
- Risk Management: Identifying and managing risks effectively to ensure the safety and well-being of service users.
- Legislation and Policy: Maintaining up-to-date knowledge of relevant legislation, including the Social Work (Scotland) Act 1968 and the Social Care (Self-Directed Support) (Scotland) Act 2013.
- Caseload Management: Effectively managing a complex caseload, prioritizing work, and maintaining accurate and timely records.
- Multi-Agency Working: Liaising with NHS colleagues, voluntary sector organisations, and independent providers.
Qualifications and Skills:
- A degree in Social Work or an equivalent qualification recognised by the Scottish Social Services Council (SSSC).
- Current SSSC registration as a social worker.
