• Conduct comprehensive assessments of individuals' needs and develop person-centered care plans.
• Provide direct support and interventions to vulnerable adults facing various challenges.
• Collaborate with multidisciplinary teams, including healthcare professionals, family members, and other stakeholders.
• Advocate for the rights and well-being of individuals, ensuring access to appropriate resources and services.
• Maintain accurate and up-to-date records of client interactions and progress.
• Participate in case reviews and contribute to the ongoing development of service delivery.
• Develop and deliver effective interventions that promote client independence and improve quality of life.
• Comply with all relevant legislation, regulations, and ethical guidelines.

We're looking for you, if:

• You hold a relevant social work qualification (e.g., Bachelor of Social Work, Master of Social Work).
• You have a minimum of 2 years of experience working with vulnerable adults in a social care setting.
• You possess a strong understanding of social work principles, theories, and practice models.
• You demonstrate excellent communication, interpersonal, and problem-solving skills.
• You are proficient in using IT systems to record and manage case information.
• You are a highly motivated and compassionate individual committed to making a positive impact on the lives of others.
• Experience working within the Adult Social Care sector would be advantageous.
