Work collaboratively with individuals, their families, carers, and other agencies to coordinate care and resources.
Advocate for the rights and needs of vulnerable adults, promoting their independence and well-being.
Manage a diverse caseload, balancing competing priorities and ensuring timely interventions.
Contribute to the ongoing development and improvement of assessment practices within the team.
Requirements:
Registered with Social Care England.
Minimum 3 years' post-qualification experience as a qualified Social Worker. Experience in conducting thorough needs assessments for adults is highly desirable.
Excellent assessment, communication, and case management skills.
Strong understanding of relevant legislation and policies relating to adult social care.
Ability to build rapport and trust with individuals and their families.
Commitment to person-centered practice and ethical standards.
