Rate: £36.65 per hour
About the role: We are seeking a skilled and compassionate Social Worker to join our team in Wiltshire. This is an integrated front-door role, meaning you will be directly involved in assessing and responding to client needs without case holding responsibilities. You will work collaboratively with other professionals to provide high-quality social work services.
Responsibilities:
- Conduct thorough assessments of client needs.
- Develop and implement care plans.
- Collaborate with other professionals to ensure integrated care.
- Maintain accurate records and documentation.
- Adhere to all relevant professional standards and legislation.
Requirements:
- Social Work Degree (or equivalent)
- HCPC registration
- Experience working within an integrated front-door setting (desirable)
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
