Position description / Job description- Conduct comprehensive needs assessments for clients.
- Develop and implement individualized care plans.
- Provide direct support and counseling to clients.
- Advocate for clients' rights and needs.
- Collaborate with other professionals to ensure comprehensive service delivery.
- Maintain accurate and up-to-date client records.
- Participate in case reviews and team meetings.
- Continuously update knowledge and skills to best serve the client population.
We're looking for you, if:
- You hold a relevant Social Work degree (e.g., Bachelor of Social Work or equivalent).
- You have experience in working with vulnerable populations and possess a strong understanding of social work principles and practices.
- You demonstrate excellent communication, interpersonal, and problem-solving skills.
- You are a highly organized and detail-oriented individual with the ability to manage multiple cases simultaneously.
- You possess a strong commitment to client advocacy and ethical practice.
- You are comfortable working independently and as part of a team.
- You are proficient in using relevant software and technology for case management.
