Position description / Job description- Conduct comprehensive assessments of clients' needs and develop individualized care plans.
- Provide direct support and counseling to clients facing various challenges.
- Collaborate with families, caregivers, and other professionals to coordinate services.
- Advocate for clients' rights and access to resources.
- Maintain accurate and up-to-date client records.
- Participate in case conferences and team meetings.
- Develop and deliver workshops and presentations on relevant social issues.
- Monitor and evaluate the effectiveness of interventions and make necessary adjustments.

- You hold a relevant social work qualification (e.g., MSW, BSW).
- You have excellent communication, interpersonal, and problem-solving skills.
- You possess a strong understanding of social work principles and practices.
- You are empathetic and compassionate, with a commitment to client well-being.
- You have experience working with vulnerable populations (advantageous).
- You are proficient in using case management software (advantageous).
- You are able to work independently and as part of a team.
- You possess a valid driver's license (advantageous).
