Responsibilities:
- Conducting holistic assessments of an individual's needs, strengths, and risks. This is often referred to as a "Community Care Assessment."
- Developing and implementing support plans tailored to the person's specific goals, such as finding a place to live, getting a job, or learning life skills.
- Regularly checking in to ensure that the support remains effective and adjusting plans as the person's needs or environment change.
- Investigating concerns of harm or neglect and taking action to protect vulnerable adults under the Adult Support and Protection (Scotland) Act 2007.
- Ensuring the individual's rights are upheld, helping them navigate complex systems, and ensuring they have a voice in decisions affecting their lives.
- Linking individuals with community resources, leisure activities, and vocational opportunities to prevent isolation.
- Helping arrange residential accommodation, home support, or respite care for family carers.
- Helping individuals manage their own care budgets to give them more choice and control over who provides their support.
- Collaborating with NHS health staff (e.g., psychiatrists, speech and language therapists, and OTs) to manage complex cases that involve both health and social care needs (e.g., epilepsy or autism).
- Acting as the bridge between the service user and other agencies like housing, voluntary organisations (Enable, Quarriers), and the Scottish Social Services Council (SSSC).
- Providing support for family members and unpaid carers, including the creation of Adult Carer Support Plans to identify their needs and offer respite.
Requirements:
- PVG
- SSSC registration
- Social Work Degree
