Responsibilities:
- You are responsible for ensuring patients leave the hospital safely and promptly once they are medically fit.
- Coordinating the transition from a hospital ward back to a person's home or a residential care setting.
- Working alongside doctors, nurses, occupational therapists, and physiotherapists to ensure the care plan is holistic.
- Identifying and solving issues that might prevent a patient from going home (e.g., lack of heating, need for home adaptations, or absence of family support).
- Conducting statutory assessments to determine what level of support a person needs under the Social Work (Scotland) Act.
- Helping patients and families navigate the four SDS options to choose how their care budget is managed.
- Focusing on "enabling" care short-term support designed to help patients regain independence so they don't require long-term services.
- Identifying and investigating concerns where an adult may be at risk of harm or self-neglect.
- For those specifically qualified (as Mental Health Officers or MHOs), duties include assessments under the Mental Health (Care and Treatment) (Scotland) Act.
- Assessing complex social situations to manage risks while respecting the individual's right to choice and independence.
- Many social workers are based directly within sites like the Mid Argyll Community Hospital & Integrated Care Centre.
- The role often requires traveling across mainland and island locations to visit patients' homes for post-discharge follow-ups.
- Acting as a link to third-sector organisations and local volunteer groups that provide essential support in isolated areas.
Requirements:
- PVG
- SSSC registration
- Social Work Degree
