The Role:
As a Social Worker within the CMHT, you will play a crucial role in assessing, planning, implementing, and evaluating care packages for individuals with complex mental health needs. You will work closely with service users, their families, carers, and a range of other professionals including psychiatrists, nurses, occupational therapists, and psychologists.
Key Responsibilities:
- Undertake comprehensive social work assessments, including risk assessments, to identify needs and strengths.
- Develop and implement person-centred care plans that promote recovery and social inclusion.
- Provide a range of therapeutic interventions and support, including crisis intervention, advocacy, and signposting to relevant services.
- Work collaboratively with service users to empower them in managing their mental health and achieving their goals.
- Contribute to the safeguarding of vulnerable adults in line with national and local policies.
- Maintain accurate and up-to-date records in accordance with professional standards.
- Participate in regular supervision, team meetings, and professional development opportunities.
Qualifications and Skills:
- A recognised Social Work qualification (e.g., BA/MA in Social Work, DipSW).
- Registration with the Scottish Social Services Council (SSSC) as a Social Worker.
- A full driving licence and access to a vehicle.
