Randstad are currently looking for Care Co-ordinators to join Pennine's CMHT team based in Tameside. This is a full time role offering a Band 6 pay rate.

Responsibilities:
- Act as the primary Care Coordinator for a designated group of service users.
- Conduct comprehensive assessments to determine the needs of service users and their carers.
- Develop, implement, and regularly review individualized care plans in collaboration with service users, families, and the multidisciplinary team.
- Monitor and evaluate care in accordance with the Care Programme Approach (CPA).
- Assess and monitor risks to service users and others, implementing appropriate risk management strategies.
- Participate in the Community Mental Health Team's (CMHT) duty system on a rota basis, providing rapid responses to immediate needs, including crises.
- Liaise with various agencies and members of the multidisciplinary team to ensure comprehensive care.
- Communicate effectively with service users, families, and professionals about complex matters, ensuring clarity and understanding.Maintain accurate, up-to-date electronic records of all service users, ensuring documentation reflects assessments, Engage in continuous professional development, attending mandatory training and participating in annual performance reviews.
- Support the development of the service, including promoting the service to other agencies and contributing to ongoing service improvements.
Requirements:
- Social Work England registered
- Social Work Degree or equivalent
- Enhanced DBS
