About the Service
Our client is registered with the Care Inspectorate as a dual Housing Support and Care at Home service. It comprises a cluster of 12 properties alongside an active community outreach service. We deliver person-centred care ranging from 25 hours per week up to 24/7 packages (including overnight support) to individuals with diverse needs, including:
- Learning disabilities and neurodivergence
- Acquired brain injuries
- Acute mental health conditions
- Substance use recovery needs
- Physical disabilities
The Role Working under the daily leadership of a Team Leader and Senior Support Worker, you will join a dynamic 15-person team delivering round-the-clock support. Your core focus will be helping individuals build independence and actively participate in their community.
Key Responsibilities
- Promote Independence: Support service users with daily living tasks, personal care, and skill development inside their homes and out in the community.
- Community Engagement: Assist individuals in participating in local activities and interests that promote health, confidence, and well-being.
- Deliver Tailored Care: Follow individualised support plans that adapt seamlessly to both cluster residents and community outreach clients with higher support needs.
- Flexible Support: Work a variety of shifts within a flexible rota to ensure continuous 24/7 care coverage, including overnight shifts.

- Adaptable: Confidence working with a wide range of support needs, from neurodivergence to complex mental health conditions.
- Strong Team Player: Excellent communication skills and the ability to collaborate effectively with colleagues, supervisors, and external health professionals.
- Relevant Experience (Desirable): Previous experience in social care, housing support, or outreach is beneficial, but strong values and a positive attitude are essential.
