Responsubiities:

• Hold a manageable caseload of children and young people with disabilities aged 0-18

• Undertake holistic assessments of need under the Children Act

• Develop and review Child in Need, Child Protection, and Short Breaks packages

• Work closely with families to develop tailored plans that promote independence, inclusion, and improved outcomes

• Carry out direct work with children and young people with a wide range of disabilities, including learning disabilities, autism, physical disabilities, and complex health needs

• Coordinate and work effectively with health, education, SEN, short breaks providers, and voluntary agencies

• Support children and families navigating transitions, including preparing for adulthood

• Prepare high-quality reports for reviews, meetings, and where required, legal processes

• Champion a strengths-based, child-focused approach
  
  

To be considered for this role, you will ideally have:

• Experience within a Children With Disabilities, Safeguarding, or other statutory children's social work team

• Knowledge of disability legislation, care planning, and multi-agency working

• Confidence in preparing clear, professional assessments and reports

• Social Work England (SWE) registration

• A BSc in Social Work or equivalent

• Excellent communication skills, emotional resilience, and a passion for supporting children with disabilities and their families
