Hospital Discharge Social Worker - Liverpool
We're currently recruiting for an experienced Social Worker to join a Hospital Discharge Team in Liverpool.
This is a fully office-based role, working within a fast-paced, supportive team focused on safe and timely discharges, effective Care Act assessments, and person-centred support planning.
It's a fantastic opportunity for a social worker who thrives in a collaborative, multi-agency environment and is confident working with adults with complex needs and health challenges.

Role Overview:
- Undertake Care Act 2014 assessments and reviews
- Support and coordinate safe hospital discharges
- Complete safeguarding enquiries and associated documentation
- Work closely with health professionals, families, and community services
- Develop person-centred, outcome-focused support plans
- Promote wellbeing, independence, and positive outcomes
 
 
Requirements:
- Qualified Social Worker (DipSW / Degree / CQSW) with SWE registration
- Experience in hospital discharge or adult social care settings
- Strong understanding of the Care Act 2014 and Mental Capacity Act 2005
- Confident working in a fast-paced, multi-disciplinary environment
- Excellent assessment, communication, and organisational skills
