Randstad are recruiting for a Qualified Social Worker to join Manchester City Council Hospital Discharge to AssessTeam. This is a full time role, offering an amazing pay rate of £30 per hour.

Responsibilities:
- Lead the coordination of safe and timely discharges for adults from hospital.
- Work as part of an integrated, multi-disciplinary team alongside NHS staff (doctors, nurses, therapists) to manage the patient's journey from admission to discharge.
- Collaborate with community teams, housing services, and other local authorities to ensure a seamless transition of care.
- Conduct comprehensive, person-centred assessments under the Care Act 2014 and the Mental Capacity Act 2005.
- For patients being discharged to assess (D2A), conduct the main social care assessment after they have returned home or to a short-term community setting.
- Review and evaluate care packages put in place by the hospital and assess for long-term support needs before a case is transferred to a community team.
- Act as an advocate for the patient's best interests, ensuring their voice and choices are at the heart of the discharge process.
- Build and maintain effective working relationships with all partners involved in the patient's care.
- Help patients and their families understand and navigate the complexities of the healthcare and social care systems.
- Identify and arrange a range of support services, such as reablement, care packages, intermediate care, and assistive technology.
- Provide advice and guidance to patients and their families on available community resources and support networks.
- Ensure all work is compliant with relevant legislation, policies, and statutory guidance.
- Maintain accurate and detailed records on social care systems to support decision-making and provide a clear audit trail.
Requirements:
- Social Work England registered
- Enhanced DBS
- Social Work Degree or equivalent
