You'll be working within an authority that promotes collaborative working, timely hospital discharges, and person-centred support planning. The role is ideal for an experienced social worker who thrives in a fast-paced environment and is confident in managing complex cases involving safeguarding and Care Act assessments.

As part of the Hospital Social Work Team, you'll:
- Undertake Care Act 2014 assessments and reviews
- Manage and coordinate safe and timely discharges
- Work collaboratively with health professionals, families, and partner agencies
- Complete safeguarding enquiries and related documentation
- Develop and implement person-centred support plans
- Ensure individuals receive the right support to promote independence and wellbeing

To be considered for the role, ideally you will have:
- Social Work Degree / Diploma / CQSW with SWE registration
- Extensive experience in Safeguarding and Care Act duties
- Proven background in hospital discharge planning
- Strong knowledge of Care Act 2014 and Mental Capacity Act 2005
- Excellent communication and partnership working skills
