Key Responsibilities
- Person-Centred Assessment & Care Planning: Conduct outcome-focused social work assessments and review care plans with adults who have learning disabilities, ensuring their choices, independence, and rights are at the heart of all interventions.
- Self-Directed Support (SDS): Guide service users and their families through Self-Directed Support options (Options 1-4) to deliver personalised care packages tailored to individual goals and lifestyle preferences.
- Statutory Duties & Safeguarding: Carry out statutory responsibilities under the Adult Support and Protection (Scotland) Act 2007 and the Adults with Incapacity (Scotland) Act 2000, conducting inquiries and applying risk-management frameworks to safeguard vulnerable adults.
- Multidisciplinary Collaboration: Work closely with health professionals (community learning disability nurses, occupational therapists, speech and language therapists), primary care teams, and local service providers within the Renfrewshire Health and Social Care Partnership (HSCP).
- Carer Support & Advocacy: Identify and support the needs of unpaid carers through Carer's Assessments, ensuring family members have access to respite, advice, and local community resources.
- Case Management & Reviews: Maintain an active caseload, producing clear, professional casework documentation, court/tribunal reports, and multi-agency review reports in line with council policies.
What We Are Looking For
- Qualified Social Worker: Degree in Social Work (DipSW, BA, or equivalent).
- Professional Registration: Active registration with the SSSC (or eligible to register immediately).
Knowledge & Experience
- Experience working with adults with learning disabilities, neurodivergence, or complex physical/sensory needs in a social work or statutory setting.
- Clear understanding of relevant Scottish legislation, including the Adult Support and Protection (Scotland) Act 2007, Adults with Incapacity (Scotland) Act 2000, and Social Care (Self-directed Support) (Scotland) Act 2013.
- Familiarity with trauma-informed practice, human-rights approaches, and national strategies such as The Keys to Life.
