Randstad are recruiting for a Qualified Social Worker to join Stockport Council's Hospital Discharge Team as a Manager. This a full time role with a fantastic pay rate of £46 per hour.

Responsibilities:
- Here are the duties for a manager role within Stockport Council's hospital team, presented as a single list of bullet points:
- Manage and supervise a large team of staff, including social workers, newly qualified social workers (ASYE), and unqualified staff.
- Provide strong leadership, support, and guidance to ensure the team delivers safe and timely hospital discharges.
- Conduct regular supervision and performance reviews for staff, promoting professional development and a strength-based culture.
- Oversee the entire hospital discharge process, ensuring a smooth and efficient transition for adults leaving the hospital.
- Ensure effective "flow" within the hospital, reducing delays and supporting the wider health system.
- Monitor team workload and capacity to ensure all cases are managed effectively under pressure.
- Maintain a thorough and up-to-date knowledge of all relevant legislation, such as the Care Act, the Mental Capacity Act (MCA), and safeguarding policies.
- Ensure the team's practice is compliant with all statutory responsibilities and regulations.
- Attend and chair safeguarding meetings, Court of Protection meetings, and other professional meetings to advocate for the best interests of service users.
- Ensure all adult social care assessments and support plans are completed in line with Care Act principles and are focused on helping people achieve their desired outcomes.
- Maximise independence for service users by considering their strengths and the assets available in the community.
- Ensure all necessary paperwork and recording processes are followed to a high standard.
- Work in close partnership with NHS colleagues at Stepping Hill Hospital, the Integrated Care Board (ICB), and other health, social care, and third-party sector partners.
- Build and maintain strong professional relationships to facilitate collaborative working and ensure a seamless patient journey.
- Manage resources effectively to control costs and drive out inefficiencies.
- Contribute to service improvement initiatives and lead on changes to policy or practice within the team.
- Demonstrate a dynamic, flexible approach and the ability to perform under pressure in a busy, demanding environment.
Requirements:
- Social Work England Registered
- Enhanced DBS
- Social Work Degree or equivalent
- Full time in office
