Responsibilities:
To provide a high-quality, person-centred social work service to looked-after young people and care leavers (typically aged 14 to 18). The primary goal is to support them through the transition to adulthood and independent living, ensuring they are safe, healthy, and prepared for a positive future.
Key Responsibilities
- Conduct statutory assessments to determine a young person's needs and aspirations.
- Develop, implement, and review individual Pathway Plans that address areas such as finance, accommodation, education, employment, and health.
- Ensure all plans are co-produced with the young person, reflecting their wishes and feelings.
- Provide practical and emotional support to help young people prepare for and move into independent living.
- Assist them in developing essential life skills, including budgeting, cooking, and household management
- Facilitate access to housing and accommodation services.
- Work closely with a range of professionals, including Personal Advisors (for care leavers aged 18+), Employability Officers, and Health Workers.Liaise with education and training providers to ensure young people have access to learning opportunities that match their potential.
- Ensure a smooth transition from the Young Persons Team to the Care Leavers Team.
- Act as a corporate parent, advocating for the young person's best interests in all aspects of their life.
- Ensure their voice is heard in all meetings and decision-making processes.
- Build and maintain trusting, supportive relationships with young people.
- Fulfill all statutory duties under Leaving Care legislation and the Children Act 1989.
- Maintain accurate, high-quality, and up-to-date case records on relevant IT systems.
- Produce reports for reviews and other statutory meetings.
Requirements:
- Social Work Qualification or equivalent
- Social Work England registered
- Enhanced DBS
