Are you an experienced and committed Registered Mental Health Nurse (RMN) ready for a challenging and deeply rewarding role?
We are seeking a specialist Clinical Nurse to join our mental health service, which focuses on providing a crucial bridge between hospital settings and independent living for young people.
Our client supports young people aged 16 to 30 experiencing complex mental health needs. These individuals often transition from long-term hospital admissions or secure care provisions, and our service offers an essential community-based alternative to inpatient care.
As the post-holder, you will be responsible for defining and delivering the specialist interventions that facilitate this transition. This involves:
Assessment: Conducting comprehensive clinical assessments of young people's needs.
Intervention Planning: Developing, outlining, and implementing specialist mental health interventions.
Pathway Delivery: Delivering care across both residential provisions and community-based independence packages.
Clinical Autonomy: Taking ownership of case management and therapeutic outcomes for complex young people.
This is a hybrid role based out of our Stockport Head Office. You will be responsible for supporting two services in Lancashire, requiring a minimum of one on-site visit per week to each location. Flexible homeworking is offered around the necessary on-site requirements.
