Are you a proactive Social Worker with a talent for navigating complex systems? We are looking for a dedicated professional to join our Hospital Discharge Team in Falkirk. This role is critical in ensuring safe, timely transitions from hospital settings back into the community, reducing delayed discharges, and improving patient outcomes.
The Role
Working at the interface of acute health and community social care, you will be responsible for coordinating comprehensive discharge plans. This is a fast-paced environment that requires quick thinking and excellent multi-agency collaboration.
Assessment: Conducting robust Section 1 assessments and community care assessments within a hospital ward environment.
Discharge Planning: Coordinating complex discharges, including Adults with Incapacity (AWI) cases and Power of Attorney (POA) involvements.
Multi-Disciplinary Working: Collaborating closely with consultants, occupational therapists, nursing staff, and families.
Risk Management: Identifying and mitigating barriers to safe discharge to prevent readmission.
Requirements
To be successful in this high-pressure environment, you must have prior experience in a similar setting.
Hospital Experience: Proven track record of working within a hospital discharge or acute social work team.
Legislation Knowledge: Strong understanding of the Social Care (Self-directed Support) (Scotland) Act and Adults with Incapacity (Scotland) Act.
Qualifications: Degree in Social Work and current SSSC registration.
Communication: Ability to negotiate and communicate effectively under pressure.
Key Details
Pay Rate: £37.00 - £42.00 per hour (Highly competitive, based on experience).
Hours: Full-time preferred, but part-time applications will be considered for the right candidate.
Location: Falkirk (Hospital Based).
Start Date: Immediate.
