Responsibilities:
- Discharge Planning & Logistics.
- Coordinating transfers to skilled nursing facilities, rehab centers, or hospice.
- Setting up home health care, physical therapy, or medical equipment (like oxygen or hospital beds).
- Linking patients with community programs like "Meals on Wheels" or transportation services.
- Clinical Assessment & Intervention.
- Evaluating a patient's mental health, support system, and living situation.
- Providing immediate support for victims of domestic violence, sexual assault, or child/elder abuse (including mandatory reporting).
- Identifying addiction issues and facilitating referrals to detox or recovery programs.
- Navigating the bureaucracy of healthcare is a nightmare for most; social workers act as the guides.
- Helping patients understand Medicare/Medicaid or apply for "charity care" for unpaid bills.
- Assisting with Power of Attorney (POA) and Living Wills so the patient's wishes are respected.
- Acting as a bridge between the medical team and the family when there is disagreement over the care plan.
Requirements:
- PVG
- SSSC registration
- Social Work Degree
