Position description / Job description- Conduct comprehensive assessments of individuals' needs and develop person-centered care plans.
- Provide direct support and interventions to individuals and families facing challenging circumstances.
- Collaborate with multidisciplinary teams to coordinate services and ensure holistic care.
- Advocate for the rights and well-being of service users.
- Maintain accurate and up-to-date records of service delivery.
- Participate in regular supervision and professional development activities.
- Engage with stakeholders to build strong relationships and partnerships.
- Contribute to the development and implementation of new initiatives and service improvements.
We're looking for you, if:
- You hold a relevant social work qualification (e.g., MSW, BA/BSc in Social Work).
- You have at least 3 years of experience in a similar role within the safeguarding sector.
- You have experience in working with vulnerable adults and children.
- You possess excellent communication, interpersonal and advocacy skills.
- You are proficient in using relevant case management software.
- You demonstrate a commitment to continuous professional development.
- You have a strong understanding of relevant legislation and safeguarding procedures.
- You are a highly motivated and compassionate individual with a passion for improving lives.
