Knowsley Council is currently seeking a highly motivated and qualified Social Worker to join its dedicated Young People's Team (YPT). Summary of the Job
The post holder will manage a diverse and demanding statutory caseload focused on adolescents and young adults, typically those aged 16+, who are looked after, care leavers, or transitioning to adulthood. The role requires a qualified professional to undertake complex assessments, develop, implement, and review robust care plans, ensuring all interventions are legally compliant and centred on the young person's voice and aspirations. They will be expected to make informed, professional judgments in high-risk situations, maintaining detailed and accurate records, and consistently adhering to the highest standards of professional social work practice. Responsibilities
Conduct comprehensive statutory assessments (including Needs Assessments and Pathway Plans) for young people and care leavers, identifying their needs, strengths, and risks.
Develop, implement, and rigorously review personalised Pathway Plans that effectively address the young person's education, training, employment, accommodation, and emotional wellbeing needs.
Provide direct, intensive social work support to young people in various settings, acting as their key worker and corporate parent, and promoting their independence and life skills.
Act as an advocate for service users, ensuring their voices are heard and rights are upheld in multi-agency meetings, reviews, and court proceedings where necessary.
Effectively manage and prioritise a complex and varied caseload, making use of excellent organisational and time management skills.
Work collaboratively with a range of internal and external agencies (including housing, health, education, and youth justice) to coordinate holistic and seamless packages of support.
Maintain accurate, timely, and evidence-based case records and reports in accordance with statutory and local authority procedures.
Participate in regular professional supervision and reflective practice sessions to ensure high-quality, safe, and ethical decision-making.
