What you'll do (Key Duties):
Key Working: Manage, compile, and review individual care plans.
Direct Care: Provide personal and intimate care with dignity.
Support: Assist with equipment, promote mental/physical activity, and ensure a high standard of living environment and personal effects.
Liaison: Work closely with health professionals, especially during illness or end-of-life care.
Documentation: Maintain accurate records and reports in line with SSSC standards.
Commitment: Work on a rota basis, including evenings and weekends, and participate in meetings and training.
