Are you an experienced Adult Social Care practitioner skilled in handling complex caseloads, forensic mental health, and multidisciplinary discharge planning?
Middlesbrough Council is seeking a high-calibre Adults' Social Worker to join a supportive, collaborative service. Offering £33.00 per hour, this role focuses on leading and coordinating specialist interventions for adults with learning disabilities, significant mental health needs, offending histories, or those at risk of entering the criminal justice system.
Key Responsibilities:
- Complex Caseload Management: Deliver person-centred support promoting recovery, independence, and public safety across complex mental health, learning disability, and forensic caseloads.
- Statutory Assessments: Undertake Care Act 2014 assessments, determine eligibility, and complete Mental Capacity Act (MCA) 2005 assessments and Best Interest decisions.
- Section 117 & Discharge Planning: Play a central role in Section 117 aftercare planning, coordinating smooth transitions from secure and inpatient settings into the community.
- Multidisciplinary & Risk Practice: Collaborate with psychiatry, psychology, nursing, and criminal justice professionals, responding to crisis situations using a trauma-informed, recovery-focused approach.
What We Are Looking For:
- Professional Registration: Active registration with Social Work England (SWE).
- Experience: Proven post-qualifying experience in Adult Social Care, particularly within Mental Health, Learning Disabilities, or forensic/criminal justice interfaces.
- Legislative Knowledge: Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983 (Section 117).
