Role Overview
An experienced, dedicated Qualified Social Worker is required to join a high-performing Hospital Discharge Team based in Sunderland. Operating within a fast-paced statutory environment, you will work collaboratively across health and social care pathways to facilitate safe, timely, and person-centered discharges for adults with complex care needs.
This role plays a vital part in managing bed flow, preventing unnecessary delays, and ensuring individuals receive the right ongoing care-whether returning home with support packages or transitioning into short-term reablement and residential settings.
Key Responsibilities
- Conduct statutory care and support assessments under the Care Act 2014, adhering to the local Discharge to Assess pathways.
- Complete Mental Capacity Assessments (MCA) and Best Interest decision-making processes where appropriate.
- Support Continuing Healthcare (CHC) screening and joint assessment processes.
- Lead on adult safeguarding enquiries arising within hospital settings, adhering to local threshold guidelines.
- Collaborate closely with multidisciplinary teams, including NHS consultants, occupational therapists, ward nurses, and community care providers.
- Manage complex caseloads and maintain accurate, timely electronic records in line with local authority compliance standards.
Requirements & Qualifications
- Qualification: Professional degree in Social Work (BA, BSc, MA, DipSW, or CQSW).
- Registration: Active registration with Social Work England (SWE).
- Experience: Minimum 2 years post-qualifying experience in Adult Social Care, preferably within a hospital, rapid response, or intermediate care environment.
- Knowledge: Expert understanding of the Care Act 2014, Mental Capacity Act 2005, Human Rights Act, and Adult Safeguarding frameworks.
- Transport: Full UK driving license and access to a reliable vehicle for community visits.
