Core Responsibilities
- Care Act Assessments & Reviews: Conduct thorough assessments and reviews under the Care Act 2014, evaluating individual needs and adjusting support packages accordingly.
- Packages of Care Management: Confidently design, implement, and amend packages of care to promote independence and safe outcomes.
- System Management: Utilize LiquidLogic / LAS to document assessments, update care plans, and record outcome data accurately.
- Person-Centred Practice: Collaborate directly with service users, carers, and multidisciplinary health partners to deliver tailored care interventions.
Mandatory Candidate Requirements
- Professional Status: Qualified Occupational Therapist registered with the HCPC.
- System Expertise: Proven familiarity and hands-on confidence using LiquidLogic (LAS).
- Core Competencies: Strong background in Adult Social Care, Care Act 2014 eligibility frameworks, and care package reviews.
- Self-Management: Highly organized with the ability to manage a flexible diary and conduct community-based assessments independently.
