About the Role:
Middlesbrough Borough Council is looking for a skilled and empathetic Social Worker to join their specialized Older Persons Mental Health Team. This is a fantastic opportunity for an adult care professional with a strong understanding of mental health conditions affecting older demographics, such as dementia, depression, and late-onset psychosis.
You will play a pivotal role in delivering specialized, high-quality care to vulnerable individuals, helping them maintain their independence and mental well-being while navigating complex cognitive and psychological challenges.
Key Responsibilities:
Undertaking specialized assessments for older individuals experiencing complex mental health issues and cognitive decline.
Creating tailored, risk-managed care plans that promote autonomy and protect vulnerable adults.
Working closely with Community Mental Health Teams (CMHTs), psychiatrists, and memory clinics to ensure holistic care.
Executing mental capacity assessments and best interest decisions where appropriate.
What We Are Looking For:
Social Work England (SWE) Registration is essential.
Solid experience working within an Adult Mental Health setting or a dedicated older persons specialist service.
In-depth understanding of the Care Act 2014, Mental Health Act 1983 (amended 2007), and the Mental Capacity Act 2005.
Exceptional crisis-management skills and the ability to build trust with vulnerable service users.
