In this role, you will champion independence and well-being, providing essential assessments under the Care Act 2014. You will collaborate with a diverse team of 32 professionals to deliver innovative care solutions for service users and their carers.
Responsibilities:- Conduct comprehensive assessments and reviews of individual needs.
- Provide expert advice on equipment, adaptations, and therapeutic techniques.
- Link service users to vital universal and voluntary resources.
- Collaborate across inter-disciplinary teams to improve service delivery.
- Diploma or degree in Occupational Therapy.
- Health Professions Council registration.
- Full driving licence.
- Minimum of 3 years post-qualification experience.
