About the Role
Are you passionate about working with young adults and helping them achieve independence? Leicester City Council is seeking an experienced Social Worker to join the Preparing for Adulthood Team.
In this rewarding role, you will support young people as they transition from children's placements into adulthood. You will focus on strengths-based practice to empower young adults, maximize their independence, and achieve positive long-term outcomes.
Key Responsibilities & Focus Areas
- Transitions Support: Guide young people through key developmental milestones and transition from children's social care into adult services.
- Assessments & Safeguarding: Complete comprehensive Care Act assessments and make confident, risk-assessed safeguarding decisions.
- Legal & Court Work: Undertake Court of Protection work and handle complex legal/safeguarding procedures where required.
- Collaboration & Funding: Work closely with families and young adults, utilizing a strong understanding of Shared Care and Continuing Healthcare (CHC).
- Strengths-Based Practice: Apply strengths-based approaches to promote dignity, choice, and independence for every young adult on your caseload.
- Out-of-Area Visits: Support young people placed within Leicester as well as those in placements located elsewhere across the UK.
Key Requirements & Experience
- Qualified Social Worker (Social Work England registered).
- Proven experience in Preparing for Adulthood and transitions.
- Hands-on experience with Care Act assessments, Safeguarding, and Court of Protection proceedings.
- In-depth understanding of Shared Care and Continuing Healthcare (CHC) frameworks.
- Strong skills in risk assessment and confident decision-making.
- Ability and willingness to travel to placements across the UK when required.
