Are you an experienced Social Work Manager skilled in transition pathways, learning disabilities, and multi-agency partnership working?
Lancashire County Council is seeking an outstanding Team Manager to provide operational leadership and management oversight to their Young Adults Team (YAT) based at County Hall, Preston. Offering £40.00 per hour, this role is key to delivering safe, seamless, and person-centred transitions for young people with care and support needs as they prepare for adulthood.
About the Role & Team:
The Young Adults Team supports young people aged 14 to 25 with learning disabilities and autism, managing the critical pathway from Children's Services into Adult Social Care. You will lead a multidisciplinary team to embed strengths-based practice, balance operational demand with staff capacity, and deliver high-quality outcomes across health, education, and social care interfaces.
Key Responsibilities:
- Operational Leadership: Provide visible management oversight, allocation planning, and leadership across a specialist multidisciplinary team.
- Transition Management: Ensure safe, timely, and effective statutory transitions under the Care Act 2014, Children & Families Act, and Mental Capacity Act (MCA) frameworks.
- Quality & Performance: Lead reflective supervision, performance management, quality assurance audits, and service improvement initiatives.
- Safeguarding & Risk: Maintain oversight of high-risk cases, adult safeguarding enquiries, and operational performance while managing team budgets for value for money.
- Multi-Agency Partnerships: Drive collaborative working with Children's Services, SEND, Health, Education, and housing providers to build holistic support plans.
What We Are Looking For:
- Qualifications & Registration: Qualified Social Worker with active Social Work England (SWE) registration.
- Transitions Expertise: Proven understanding of the transition pathway from Children's to Adult Social Care within Learning Disabilities & Autism settings.
- Leadership Skills: Demonstrated track record in providing effective supervision, managing budgets, and balancing complex demand and risk.
- Legislative Knowledge: Expert understanding of the Care Act 2014, MCA 2005, and adult/children safeguarding legislation.
