You will be required to have experience of working in a similar setting and role. You will undertake the duties of care coordinator, managing a caseload of service users, with complex mental health needs, under CPA framework. The Team consists of experienced multidisciplinary colleagues, including Psychiatrists, Senior Nursing Practitioners, Occupational Therapists and support workers, all working to a Recovery-based approach. You will have ongoing supervision, with opportunities to develop via relevant training, appropriate to this role. The Team currently provides a Mon to Fri, 9~5 service. Our office base is The Bluebell Centre, Huyton.
