The post holder will also work with Housing Options to establish priority need relating to medical circumstances and also identify at the earliest point, suitable accommodation in a timely manner. The post holder will provide support in referrals to Doncaster Council OT services and embed effective referral paths betweenagencies in accessing housing, effective and responsive service delivery and promotingindependent living.To provide a timely assessment of customers medical needs.* To work with the Home Options Team to assist in establishing priority needin relation to medical circumstances at the earliest opportunity. To ensureaccommodation is suitable to meet the customers need when discharging theauthority's duty. This will include temporary and permanent accommodation with amore timely move through to permanent accommodation.* To provide timely advice and support in referrals to the Doncaster Council'sOT services for adaptations in order to prevent homelessness.* To be involved in a partnership approach with planned hospital discharges toavoid unnecessary delays.* To work with Doncaster Council to ensure applicants are supported throughthe allocations process and given clear information on appropriate accommodationand realistic rehousing advice.* To contribute to the efficient and effective use of St Leger Homes resourcesthrough, e.g. adherence to relevant policies and procedures regarding, recordkeeping, equipment, adaptations, identifying and recording unmet needs for futureplanning purposes.* Liaise with the Partner agencies including, Social Care, Health, Housingadaptations, officers from other authorities, medical staff including GPs, consultantsand nursing staff, probation, councillors, and supported housing providers toestablish and embed effective referral paths.* Assess the needs of vulnerable people and their carers, and to provideand/or facilitate provision of appropriate occupational therapy intervention, e.g.advice, specific treatments, equipment trial/provision, adaptations,recommendations, counselling and supporting service users and carers in themanagement of disability.* Administrative duties including responding to email, answering enquiries,telephone calls, inputting applicant details on to the IT systems.
